1. SMRT has studied the Committee of Inquiry (COI)
report.
2. The December incidents were caused by a confluence of
factors, a key one of which was isolated and latent material
defects, in particular, in a third rail fastener, and the
coincidental existence of two adjacent insulators with
defects. The various factors caused a claw to drop, the third
rail to sag and the situation to aggravate. The 17 December
incident is in turn a result of the 15 December incident, due
mainly to one or more possible "rogue trains" that suffered
not easily detectable CCD shoe damage from the 15 December
incident causing claws to drop and the third rail to sag.
3. SMRT has operated a comprehensive maintenance regime in
the past, regularly validated by LTA, which has served us
well and placed SMRT amongst the top performing metro
operators, as evidenced by international benchmarking studies
and testimonies from the experts at the COI hearings.
4. Clear challenges arise from the aging of the MRT system
and higher strain from increased train runs to cater to
rapidly increasing ridership. Whilst SMRT has been trying to
address this, quite clearly, there are areas we can improve
in our maintenance and monitoring regime and our work
processes.
5. Since the December incidents, we have been implementing
various initiatives to improve service reliability and
incidence response. We have been doing this in

collaboration with LTA. The improvements included many of the
recommendations set out in the COI report. In some other
areas, we have even gone beyond COI recommendations. We will
review the COI recommendations to take in further enhancement
measures as appropriate, in conjunction with LTA.
6. SMRT wishes to thank the commuters for their patience and
understanding as we work at our improvements. We will improve
and serve the commuters better.
