This form is created within the frame of the 13th article of the Personal Data Protection Law no 6698 ("Law") and it is filled for the purposes of exercising the data subject's right to apply to MB Tech Mühendislik ve Danışmanlık Limited Şirketi ("MBtech") which acts in the capacity of data controller. This Application Form should be used in cases where the processing is subject to the Law and the data subject would like to exercise his or her rights under Article 11 of the Law to MbTech. In case the processing is not subject to the Law, this Application Form shall not be used. Your personal data contained in this form shall be processed within the framework of the Article 5/2-ç ("processing is necessary for compliance with a legal obligation to which the data controller is subject") of the relevant Law and shall be protected under the same Law.

This application form you filled is issued for the replies related to your personal data processed by MBtech, in an accurate manner and in due time, following the determination of your relation with MBtech. MBtech reserves the right to request additional documents and information (identification card, passport, power of attorney copy etc.) in order to eliminate the risks which may arise from illegal and unfair data sharing and specifically to provide the security of your personal data.

In case the information you submitted in the form is not true or up to date or if an unauthorized application is made, then MBtech is not liable for the claims attributable to the mentioned incorrect information of unauthorized applications or the problems that may arise during the submittal of our replies to the addresses you have given.
