Report topic:Conclusion of an amendment to the contract - as part of a package of contracts with the NFZ (National Health Fund)
Report content:The Board of Directors of POLMED S.A. with its registered office in Starogard Gdański makes public that on 4 January 2016 the Issuer received a signed amendment to the contract for the provision of health care services concluded with the National Health Fund - the Pomorskie Voivodship Branch in Gdańsk (hereinafter: the NFZ). The value of benefits in 2016 resulting from the amended contract is estimated by the Board of Directors at PLN 1,625,000.
The amendment No. 19/2015 to the contract no. 11/00865/POZ-NS/13/15 for the provision of after hours primary health care services extends the contract period by 6 months to 30 June 2016 and establishes the maximum amount of obligations of the NFZ to the Issuer for the execution of the contract during this period at PLN 1,624,800 (one million six hundred and twenty-four thousand eight hundred Polish zlotys).
Other conditions of the amended contract do not differ from the commonly used conditions for these types of contracts.
The criterion for acceptance that the abovementioned Contract is significant is the fact that the Issuer in the last 12 months signed more contracts or amendments to the contracts with the NFZ branches, which together meet the criterion of a significant contract, i.e. exceed 10% of the equity capital of the Issuer.
The total value of contracts and amendments to the contracts for the provision of health care services signed with the NFZ for 2016 is estimated by the Board of
the Issuer at PLN 3,297,000. However, it should be noted that this is only an estimated amount relating only to the small part of the whole package of contracts because amendments to other contracts with the NFZ branches, setting out the maximum value of benefits for 2016 or capitation rates for other types of contracts, have not been concluded yet. The Issuer will inform about next contracts forthwith.
